These drugs are extremely dangerous. I’m surprised you recommend them at all.

Besides the temporary benefits of weight loss there are many long-term side effects that can be quite damaging. I wrote about my own experience with Ozempic here:

Knocking saturated fats? Isn't that how the 1992 food pyramid initiated the obesity epidemic?

Back when these were just diabetes drugs, a specialist put me on one. I can see why it would help with weight loss as, almost immediately, all food and my cigars tasted terrible. I would also have a big welt at the injection site. The doctor said that was nothing and it would go away. After 5 weeks, I had 5 welts and none of them were going away! I quit then and went back to good old metformin. My belly and my taste buds thanked me profusely!

But can people simply take this drug the rest of their lives and not have to do any special diet or exercise? Any problem with that? I’ve read this drug has some bad side effects. Obesity may be declared a disease so insurance and all of us paying for insurance will have to cover the lifetime costs of this drug for many people.

Incretin use is a weight loss diet, just like fasting, bariatric surgery, and the keto diet (ketosis); but like any weight loss diet, once you lose weight, you must change your diet. If you go back to the diet that made you fat to start with, you will get fat again. Best to avoid getting fat (insulin resistance) to begin with. If you get fat, you must lose the weight. You have a history of insulin resistance, thus go on a lower carbohydrate diet (not ketosis) than the normal population.

The future of GLP will involve oral versions. Although not as potent (these medications are dose related),they will be much cheaper There will be generics made in India and China, and you can ship them through the mail.

If you use your common sense and either quit the medicines, or lower the dose if you are having side effects, they are much cheaper.

Obesity in children often starts in the womb (Epigenetics and Pregnancy: Fat Newborns and Kids) Frequent and rapid elevations of insulin from frequent and rapid elevations of glucose produces epigenetic changes that result in insulin resistance in both children and adults. There are several factors involved, but RGK is correct. It is our food.

Researchers at Massachusetts Eye & Ear reported upon an alarming, near 7-fold increase in relative risk for non-arteritic ischemic optic neuropathy--with potentially devastating impact on the vision of an affected eye--in individuals treated with GLP-1 agonists: doi: 10.1001/jamaophthalmol.2024.2296 There is still much uncertainty as to a mechanism for this apparent finding and what degree of risk it may or may not suggest for individuals, but one important consideration is the receptors for any hormone or neurotransmitter in the body are susceptible to up or down-regulation, based upon the levels of the molecule present. While natural GLP-1 shows clear neuro-protective impact, it has a relatively short half-life of "about 2 minutes" vs. the pharmaceutical agonists, with half-lives reported 3-13 hours, per online search. Do we know if the extended presence/increased half-life of these medications may down-regulate receptor levels over time? Issues along these lines have been studied for another naturally short-acting hormone, erythropoietin, and suggest such concern may be indicated: https://pmc.ncbi.nlm.nih.gov/articles/PMC3587987/ Our steps in attempts to "improve upon Nature" must be taken with great caution, and these rapid rebound weight-gain experiences may need to be investigated beyond just presumption of resumed diet and excercise deficits.

These peptides are beneficial for some people at the correct dosages and with lifestyle changes. They should not be thrown out with the bathwater.
