That NIH paper is a joke. I had a very rough case of "Covid" at 63 years old. I was desperate to get out patient care and literally had to tell my doctor I would die at home before going to the hospital. My O2 was high 70's low 80's. I was able to get on-line ivermectin and eventually my doctor prescribed Azithromycin, Methylprednisolone, and a albuterol inhaler. This in addition to Vit D, C and Zinc and I was out of the woods in 4 days. Also I was already on a blood thinner.
Your protocol guideline was a life saver. I didn't follow it exactly, but as best I could, and it worked.

Why isn't ivermectin available OTC in the US? It is in most other countries.
